Requires employers to provide each employee with a total job compensation statement at the time of wage payment.

Statement must be in writing, either as a detachable part of the check/voucher or a separate document for cash/personal check payments.

Statement must include an itemized list of gross wages earned and net wages earned.

Statement must itemize the employer's and employee's cost for medical, life insurance, long-term disability insurance, retirement, social security, Medicare, unemployment insurance, and workers' compensation benefits.
