Amends Arizona Revised Statutes Section 42-2151 to require a business ethics pledge on the combined business identification and licensing form.

The Department of Revenue and Department of Economic Security shall cooperatively provide a single form for businesses to obtain employer identification numbers, transaction privilege tax licenses, and other identification numbers and licenses.

Requires businesses to acknowledge a pledge to operate within generally accepted principles of good business ethics, including honesty, fairness, and legal and moral standards.

The Department of Revenue shall make the form available at its office or by mail, specify all necessary fees, process forms and forward copies to the Department of Economic Security, and deposit all fee revenues in the state general fund.
