Establishes registered domestic partnerships in Arizona for two unrelated adults in a committed relationship who share a household, requiring both parties to be at least 18 years old, not married, and competent to contract.

Partnership registration requires filing a notarized affidavit with the superior court clerk in either applicant's county of residence or in Maricopa or Pima County, with filing fees matching those for marriage licenses under section 12-284.

Limits registered domestic partnership benefits to hospital visitation, emergency medical decision-making, inheritance, burial, and cremation rights only—explicitly stating these are the only responsibilities and benefits equivalent to those prescribed in title 25 (marriage law).

Allows registered domestic partners to modify partnership terms through premarital agreements and to dissolve partnerships using the same procedures and fees as divorce proceedings under title 25, chapter 3.

Amends section 36-3231 to include registered domestic partners in the priority order of surrogate decision-makers for health care treatment decisions when a patient is unable to communicate.
