Requires AHCCCS applicants and those seeking redetermination of eligibility to identify their employer and their spouse's employer; for child applicants, the parent or responsible party must identify their employer.

Mandates the AHCCCS administration prepare an annual report by November 15 listing each employer identified, total number of employees at each employer, and number of those employees and their spouses and dependents enrolled in AHCCCS.

Requires submission of the report to the governor, president of the senate, speaker of the house of representatives, and secretary of state, with the report designated as a public document.

Defines "employee" as a person compensated by an employer on a full-time, part-time, or seasonal basis.

Limits employer reporting requirements to businesses with at least 250 employees in Arizona.
