Establishes automatic consent to organ donation for persons applying for a driver license, permit, or nonoperating identification license in Arizona unless they affirmatively opt out during the application process.

Requires the Department of Motor Vehicles to establish procedures allowing applicants to indicate their preference not to be an organ donor during the application process.

Permits release of driving record information for persons who consent to organ donation for the purpose of registering them in the donor registry maintained under Arizona Revised Statutes section 36-858.

Prohibits implementation of this automatic donation provision unless the state's organ procurement organization provides adequate funding to the Department of Motor Vehicles to cover all implementation costs.

Defines "document of gift" by reference to existing Arizona law (section 36-841).
