Establishes a statewide human services district governed by a five-member board appointed by the governor, house speaker, senate president, and minority leaders of both chambers, each serving five-year terms.

District authorized to levy a liquor luxury tax of $4.00 per gallon of spirituous, vinous, or malt liquor, with revenues collected monthly by the state treasurer and remitted to the district treasurer.

District revenues designated to fund state programs for domestic violence, mental health, children's health care, services for the developmentally disabled, child protective services, and the state's portion of Medicaid (Title XIX) behavioral health care costs.

Board empowered to adopt corporate seal, sue and be sued, enter contracts and intergovernmental agreements, maintain public proceedings records, and elect internal officers including chairperson, secretary, and treasurer.

Initial board member terms staggered from January 1, 2012 through January 1, 2016, with subsequent appointments following the standard five-year term structure.
