Amends Arizona license plate laws to establish four new special plate programs: multiple sclerosis awareness, Arizona masonic fraternity, hunger relief, and childhood cancer research, each requiring $32,000 in advance funding by specified deadlines.

Sets special plate fee structure at $25 annually with $8 designated as administration fee (deposited to state highway fund) and $17 as donation (directed to designated charitable funds or causes).

Creates two new state funds: multiple sclerosis awareness fund (section 36-120) and childhood cancer and rare childhood disease research fund (section 36-121) to receive and distribute donations from respective license plate programs.

Authorizes centennial commemorative license plates through December 31, 2012 to commemorate Arizona's statehood centennial, with fees deposited to centennial 2012 foundation fund for tourism promotion.

Modifies obscured state name provision on license plates to apply to "the state that issues the plate" rather than specifically "this state," and changes civil penalty structure for violations from single amount to graduated penalties ($30 for first violation, $100 for repeat violations within 12 months).
