Requires advertisements for health care services that include a health professional's name to identify the professional's license title and type under which they are practicing.

Defines "advertisement" to include billboards, brochures, pamphlets, radio and television scripts, electronic media, printed directories, telephone and direct mail solicitations, and other promotional means intended to induce people to enter into service agreements.

Excludes from the definition of advertisement any materials about network providers created by entities regulated under title 20.

Establishes that violation of these disclosure requirements constitutes unprofessional conduct subject to disciplinary action.
