Requires restaurant license applicants renewing their licenses to provide documentation to the Department showing the percentage of gross revenue from food sales versus spirituous liquor sales submitted to their insurance carrier.

Imposes a civil penalty of not less than $35 for failure to provide the required insurance documentation, with all penalty monies retained by the department.

Defines "gross revenue" as revenue from all food and liquor sales on licensed premises, regardless of license type.

Maintains existing definition of "restaurant" as an establishment deriving at least 40% of gross revenue from food sales, with off-premises food sales capped at 15% of total gross revenue.
