Requires hospitals and facilities in Arizona performing abortions to report each procedure to the Department of Health Services using a form prescribed by the department, with reports submitted within 15 days of the end of each reporting month.

Reports must include detailed information about each abortion including patient age, education, residence, race/ethnicity, marital status, pregnancy/abortion history, gestational age, reason for abortion, procedure type, and any medical complications, but must not identify the patient by name or common identifiers.

Requires health professionals treating women with complications from abortion to file separate complication reports with the Department of Health Services containing information about the abortion, the nature of complications, and medical treatment provided.

Department of Health Services must prepare an annual statistical report based on collected data and make it publicly available; reports themselves remain confidential except for disclosure to law enforcement with court order or to medical boards for disciplinary purposes.

Establishes criminal and professional penalties for violations including class 3 misdemeanor for unauthorized disclosure of report information, class 1 misdemeanor for filing false reports, license suspension or revocation for failure to file required reports, and civil penalties for facilities that wilfully violate reporting requirements.
