Requires national political party committees to submit written notice to the Arizona Secretary of State of their presidential and vice-presidential nominee nominations.

Mandates that within ten days after submitting candidate names, the national political party committee must submit an affidavit signed by the presidential candidate stating citizenship, age, and residency qualifications.

Requires the affidavit to be accompanied by documents proving the candidate is a natural born citizen, proving the candidate's age, and proving the candidate meets the residency requirements for President under the U.S. Constitution.

Authorizes the Secretary of State to review the affidavit and supporting documents and prohibits placing a candidate's name on the ballot if the Secretary has reasonable cause to believe the candidate fails to meet citizenship, age, or residency requirements.
