Amends Arizona's voter registration form requirements under section 16-152, Arizona Revised Statutes.

Adds a new requirement (subsection F) that voter registration forms must include a space for registrants to provide the name of any person who assisted them in registering to vote, including that person's organization if applicable.

Does not modify any other existing voter registration form requirements, which include citizenship verification, age confirmation, felony conviction status, and residency attestations.
