Requires state agencies and political subdivisions (including school districts, charter schools, community college districts, and universities under Arizona Board of Regents) to include a "multiracial" category when surveying ethnicity if required by law or necessary to implement a legally required program.

Applies only to government forms created after the bill's effective date; existing forms may continue to be used until depleted.

Mandates that ethnicity survey lists include multiracial as a distinct category option alongside other ethnic categories.
