Amends Arizona Revised Statutes Section 28-669 regarding accident report forms.

Requires the Department of Public Safety to prepare and supply accident report forms to police departments, sheriffs, and other appropriate agencies.

Specifies that written reports by investigating officers must contain sufficiently detailed information about the cause, existing conditions, and persons and vehicles involved in traffic accidents.

Requires accident reports to be made on department-approved forms containing all required information unless unavailable.

Mandates that the Department modify written accident report forms in conjunction with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
