Requires candidates to provide an original long-form birth certificate with specific details including date, place of birth, hospital, attending physician, and witness signatures as part of the affidavit.

Requires sworn statements attesting that the candidate has not held dual or multiple citizenship and identifying all U.S. residences for the preceding fourteen years.

Prohibits the Secretary of State from placing a presidential candidate's name on the Arizona ballot if the candidate and national political party committee fail to submit and swear to the required documents.
