Amends Arizona Revised Statutes section 23-1070 to establish a pilot program allowing the state and either a city with population over 150,000 or county with population over 1 million to provide medical, surgical and hospital benefits to workers' compensation claimants.

Qualified cities or counties must apply to the industrial commission on or before September 30, 2011, with selection to occur by October 31, 2011.

Participating state and local entities must comply with procurement requirements for medical networks to direct care but are otherwise exempt from restrictions on providing these benefits.

State and selected city or county must report results to the joint legislative budget committee by December 31, 2013, including average cost per claim and average time off work for injured workers.

Pilot program terminates on December 31, 2014, after which the state and selected city or county may no longer provide medical, surgical and hospital benefits under this authority.
