Establishes the Arizona Health Insurance Exchange as a marketplace for individuals and small businesses to purchase qualified health plans, effective January 1, 2014.

Creates an 11-member Exchange Board of Directors appointed by the governor, including representatives from health carriers, dental carriers, insurance producers, small businesses, and individuals, with the director of insurance serving as a non-voting member.

Requires the Exchange to certify health benefit plans meeting federal standards, facilitate enrollment, provide consumer information, pay health insurance producer commissions, and operate a toll-free hotline and website for plan comparison.

Allows the Exchange to charge assessments or user fees to health carriers beginning January 1, 2015, to become self-sustaining, with fees disclosed separately to consumers.

Includes a conditional repeal provision if the federal Patient Protection and Affordable Care Act (P.L. 111-148) or its Section 1311 is declared unconstitutional or repealed by Congress.
