Report must include the names and addresses of defendants, plaintiffs, and registrants providing services, plus a statement describing the nature of the occurrence resulting in the malpractice action.

Report is not discoverable and not admissible as evidence in legal proceedings.

Board must initiate an investigation upon receipt of the report to determine if the registrant violated any provision of the statute or adopted rules.

Makes technical corrections to language regarding registrant information (changes "names" and "addresses" to singular form) and rule terminology (changes "promulgated" to "adopted").
