Requires health insurance contracts entered into or renewed on or after January 1, 2012 to reimburse health care providers for acquisition costs of child and adolescent vaccines recommended by the CDC's Advisory Committee on Immunization Practices at CDC private sector cost plus 21% minimum for shipping, handling and storage.

Mandates reimbursement for total vaccine administration costs including supplies, data entry, counseling, inventory management and nursing activities based on current year Medicare rates published by CMS.

Prohibits health insurance plans from imposing deductibles, copayments, or other cost sharing for recommended child and adolescent vaccines, and prohibits placing dollar limits on immunization coverage.

Requires health insurance plans to update enrollee benefits within 30 days of CDC vaccine recommendation changes and update reimbursement amounts within 45 days of pricing changes, with reimbursement to providers due within 45 days of immunization documentation.

Applies requirements to HMOs, health care services organizations, disability insurers, group disability insurers, blanket disability insurers, and accountable health plans, but excludes limited benefit coverage.
