Establishes definitions for donor-conceived children, donated eggs, donated sperm, gamete donors, and health facilities in Arizona law.

Requires health facilities to compile and provide prospective parents with detailed written nonidentifying health and genetic history information about gamete donors before creating human embryos.

Mandates health facilities retain records containing donor health and genetic history for 99 years, with records transferred to the Department of Economic Security if a facility closes operations.

Makes donor health and genetic history records available to parents, donor-conceived children age 18 and older, gamete donors, and specified family members throughout the 99-year retention period.

Requires persons requesting access to donor information to pay the costs associated with providing that information.
