Requires national political party committees to submit written notice to the Arizona Secretary of State of their presidential and vice-presidential candidates.

Mandates that within 10 days of submitting candidate names, the national party committee must submit an affidavit from the presidential candidate stating citizenship, age, and proof of meeting constitutional requirements for the presidency.

Requires the affidavit to include an original long-form birth certificate, sworn statements that both parents were U.S. citizens at birth, sworn statement of sole U.S. allegiance with no dual citizenship, and documentation of U.S. residency for the preceding 14 years.

Prohibits the Secretary of State from placing a presidential candidate's name on the Arizona ballot if the candidate and national party committee fail to submit and swear to the required documents.
