• Health professionals must provide patients or their agents with an estimate of charges for health care services within three business days of request, including discounts and applicable medical codes.

• Health care insurers and self-insured health plans must provide coverage summaries within three business days showing estimated out-of-pocket costs (deductibles, copayments, coinsurance), estimated plan payment amounts, and whether costs exceed allowable charges under the plan.

• Estimates provided by health professionals must represent good faith efforts to provide accurate information and include the identity of the provider(s), facility address, and applicable medical codes to allow patients to check insurance coverage.

• Insurers must provide coverage information in writing, orally, or electronically as directed by the insured and must make good faith efforts to provide accurate information.

• All estimates are explicitly not binding contracts or guarantees of actual charges or payments.
