Amends Arizona Revised Statutes Section 28-669 regarding accident report forms prepared and supplied by the department to police departments, sheriffs, and other agencies.

Modifies language in subsection A to clarify that written reports "required" to be made by investigating officers must contain detailed information about traffic accident cause, existing conditions, and persons and vehicles involved.

Requires accident reports to be made on department-approved forms containing all required information unless such information is unavailable.

Mandates that the department modify the written accident report form in coordination with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
