Amends Arizona Revised Statutes Section 28-669 regarding accident report forms prepared and supplied by the department.

Requires written reports by investigating officers to call for sufficiently detailed information about traffic accidents including cause, existing conditions, and persons and vehicles involved.

Specifies that accident reports must be made on department-approved forms containing all required information unless information is unavailable.

Authorizes the department to modify the written accident report form in consultation with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
