Makes the required report non-discoverable and inadmissible as evidence in legal proceedings.

Changes statutory language from "names and addresses" to "name and address" (singular) in Section B.3 regarding registrant information.

Changes terminology from "promulgated" to "adopted" when referring to board rules in Section D.
