Requires state agencies and political subdivisions to include a multiracial category on surveys that determine ethnicity when such surveys are required by law or deemed necessary to implement a state-mandated program.

Applies to public officers in agencies, school districts, charter schools, community college districts, and universities under the Arizona Board of Regents.

Takes effect only on governmental forms created after the law's effective date; agencies may continue using existing survey forms without the multiracial category until those forms are depleted.
