Allows physicians performing workers' compensation medical examinations to disclose controlled substance prescription monitoring data from the Arizona State Board of Pharmacy to the employee, employer, insurance carrier, and commission.

Requires physicians to report on off-label use of narcotics and schedule II controlled substances, high-dose opioid prescriptions (120+ milligrams morphine equivalent per day), and long-acting opioids prescribed for acute pain when requested by interested parties.

Mandates that physicians provide justification for controlled substance use and include in treatment plans measures to monitor and prevent abuse, dependence, addiction, or diversion.

Permits employers, carriers, or the commission to request prescription monitoring program inquiries and require physician compliance regardless of prior awards for medical maintenance benefits.

Authorizes the State Board of Pharmacy to release controlled substance prescription data to physicians performing workers' compensation evaluations under section 23-1026.
