Amends Arizona Revised Statutes Section 28-669 regarding accident report forms prepared by the Department of Transportation.

Requires accident report forms to call for sufficiently detailed information about traffic accidents including cause, existing conditions, and persons and vehicles involved.

Specifies that written accident reports must be made on department-approved forms containing all required information unless such information is unavailable.

Directs the department to modify written accident report forms in coordination with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
