Amends the referendum petition form requirements in Arizona Revised Statutes Section 19-101 to change the signature sheet layout from 15 lines to 10 rows of boxes.

Requires petition sheets to measure approximately 8.5 by 11 inches and be printed in landscape format with clearly labeled boxes for signature, date, printed name, street address or residence location description, city or town, zip code, and optional email address.

Mandates that circulators must declare at the top of each petition sheet in 12-point bold capital letters whether they are a "paid circulator" or "volunteer" before circulating the petition.

Voids any signatures obtained in violation of the paid/volunteer circulator disclosure requirement, though invalid signatures do not invalidate other valid signatures on the same petition.
