Amends Arizona Revised Statutes section 28-669 to require accident report forms to document whether persons involved in traffic accidents were engaged in distracted driving in violation of section 28-913.

Adds new section 28-913 to prohibit persons from operating motor vehicles on highways while distracted in any manner.

Authorizes law enforcement officers to issue citations for distracted driving violations when the officer believes the distracted driving is hazardous to public safety.

Requires coordination between the Department of Transportation and state and local law enforcement agencies, engineering agencies, and emergency medical service organizations to modify written accident report forms accordingly.
