Amends Arizona workers' compensation law to allow employers and insurers to form or contract with workers' compensation health care organizations to provide medical, surgical, and hospital benefits to injured employees through provider networks.

Requires employers to notify employees within 10 days of contracting with a health care organization, including a list of providers, toll-free number, website access, and explanation of employee rights regarding medical treatment changes.

Prohibits employees from changing medical providers outside the contracted network without written employer or insurer authorization; employees may request commission approval to change providers only if their health, life, or recovery is endangered or impaired.

Allows employers, insurers, and health care organizations to negotiate reimbursement rates different from the state fee schedule; services must not require deductibles, copayments, or other financial contributions from employees.

Effective December 31, 2012; applies to state entities, political subdivisions, insurance carriers, and certain self-insured employers.
