Amends Arizona Revised Statutes sections 12-2293, 12-2294, 36-470, 36-2525, 36-3801, and 36-3804 relating to health information privacy and access.

Allows patients and health care decision makers to receive medical records and clinical laboratory results directly from health care providers, with limited exceptions for safety or confidentiality concerns.

Permits electronic transmission of controlled substance prescriptions (schedules II-V) when the prescribing practitioner is properly registered and licensed.

Adds definition of "de-identified health information" and clarifies that individuals have the right to opt out of health information organizations to prevent sharing of their individually identifiable health information.

Requires health information organizations to maintain written notice of their practices and obtain documented patient consent before enrolling patients in the organization, with opt-out requests taking effect within 30 days.
