Amends Arizona Revised Statutes section 20-466.03 to require insurance claim forms include a notice statement about penalties for false or fraudulent claims.

Requires the notice statement to appear in at least 12-point type and state: "Any person who knowingly presents a false or fraudulent claim for payment of a loss is subject to criminal and civil penalties."

Defines "claims form" as any document supplied by an insurer that an insured, claimant, or other person is required to complete and submit in support of a claim for benefits.

Applies the requirement to all claim forms provided by insurers to insureds or any other person filing notices or making claims on insurance policies or contracts.
