• Amends Arizona health insurance statutes (sections 20-826, 20-1057.08, 20-1402, 20-1404, and 20-2329) to modify contraceptive coverage requirements for health insurance plans.

• Replaces "religious employer" exemption with broader "religiously affiliated employer" definition that includes entities whose articles of incorporation identify them as religiously motivated organizations, in addition to nonprofits meeting Internal Revenue Code section 6033(a)(3)(A) requirements.

• Expands contraceptive exemption to allow religiously affiliated employers to exclude coverage for "specific items or services" rather than just all contraceptive methods, including items related to contraceptive, abortifacient, abortion, or sterilization purposes.

• Adds protections prohibiting religiously affiliated employers from obtaining employee protected health information in violation of HIPAA and clarifies exemptions do not restrict employment discrimination protections under federal or state law.

• Applies to all health insurance contracts, policies, and evidence of coverage issued or renewed on or after the effective date of the legislation.
