Adds Chapter 23 to Title 12 of Arizona Revised Statutes to establish a legal framework for civil unions between two eligible persons.

Requires applicants to be at least 18 years old, not related by blood in disqualifying ways, not currently married, and competent to contract; they must submit a notarized affidavit to a superior court clerk who issues a certificate of civil union.

Grants parties to a civil union the same legal responsibilities and benefits as married couples, including community property, inheritance rights, guardianship, conservatorship, life insurance, burial, cremation, and adoption rights.

Permits civil unions to be modified through agreements similar to premarital agreements and dissolved through the same legal process as divorces under Title 25, Chapter 3.

Protects religious organizations from being required to participate in formalizing civil unions while allowing persons authorized to solemnize marriages under Arizona law to formalize civil unions.
