Establishes the Arizona Health Insurance Exchange, a marketplace for purchasing qualified health plans by individuals and small employers, with operations beginning January 1, 2014.

Creates a 9-member Arizona Health Insurance Exchange Board composed of the directors of the Department of Insurance and Arizona Health Care Cost Containment System Administration, three gubernatorial appointees, and four legislative appointees, with specific expertise requirements.

Requires health insurers offering plans through the exchange to provide plans at multiple coverage levels (bronze, silver, gold), charge the same rates on and off the exchange, and comply with federal certification standards and transparency requirements.

Establishes the Arizona Health Insurance Exchange Fund from federal planning grants and premium assessments to support exchange operations, and authorizes the board to charge assessments or user fees to health carriers and premium surcharges to qualified purchasers beginning January 1, 2014.

Directs the exchange to provide consumer assistance including toll-free hotlines, comparative plan information online, premium tax credit calculators, and a navigator program to educate the public and facilitate enrollment in qualified health plans.
