Amends Arizona Revised Statutes Section 36-889 regarding child care facility licensing requirements.

Changes language in subsection B from "or other state" to "or another state" as a technical correction to improve grammatical clarity.

Requires applicants and licensees to maintain a list of controlling persons on file with the department and affirm that no controlling person has been denied a child care license or certificate, or had one revoked for endangering children's health and safety.

Mandates applicants and licensees notify the department within 30 days of any changes to officers, directors, or controlling persons and provide their names and addresses.

Requires each applicant or licensee to designate an agent authorized to receive department communications and legal service of process, who must be a controlling person, U.S. citizen or legal resident alien, and state resident.
