Authorizes the Department of Transportation to issue service club special plates if an entity pays $32,000 by December 31, 2012 for implementation costs, with the entity designing the plates subject to department approval.

Establishes a $25 annual fee for service club special plates, consisting of $8 in special plate administration fees (deposited to state highway fund) and $17 as annual donations (deposited to service club special plate fund).

Creates the service club special plate fund administered by the director, with the first $32,000 reimbursed to the implementing entity, up to 10% annually used for administrative costs, and remaining monies distributed annually to a selected qualifying entity.

Requires the director to select a service club entity that has existed for over 100 years, has over one million members, and promotes service ideals, high ethical standards, and international understanding and goodwill.

Allows service club special plates to be combined with personalized special plates at the request of vehicle owners, subject to applicable fees for both types of plates.
