Prohibits health care insurers and third-party payors from requiring health care providers to participate in Medicare, Medicaid, CHIP, or health benefit exchange programs as a condition of network participation or service contracts.

Defines "health care facility" to include hospitals, outpatient surgical centers, diagnostic imaging centers, urgent care centers, nursing facilities, and rehabilitation centers.

Defines "health care provider" to include health care facilities and health care professionals licensed under Arizona Revised Statutes title 32, chapters 13, 15, 17, or 25.
