Establishes a uniform prior authorization form for prescription drugs to be used by all health care insurers beginning January 1, 2014.

Requires the Department of Insurance to convene a committee by September 1, 2013 to develop a standardized form that is no more than two pages and usable in both written and electronic formats.

Deems prior authorization requests automatically granted if a health care insurer fails to respond within five business days or refuses to use the standard form.

Allows authorized agents of health care professionals (employees, contract employees, or authorized pharmacists) to initiate and manage prior authorization requests on behalf of prescribers.

Applies to all health care insurers providing prescription drug benefits and all licensed health care professionals authorized to prescribe drugs in Arizona.
