Requires the Department to conduct and submit a full impact analysis to the Commission before applying rotenone or antimycin A to any water body in Arizona, assessing risks to the environment, humans, livestock, and downstream potable and livestock water sources.

Commission must issue written approval only if it finds the application will not endanger environmental or human health, or written disapproval of the analysis.

Department must provide at least 10 days' advance written notice to all owners, lessees, and permittees of contiguous property and publish general notice in two local newspapers before application.

Department must conduct soil and water analyses before application to establish baseline levels and continue monitoring downstream levels after application until they return to pretreatment baseline levels.

Exempts applications to reservoirs with at least 10,000 acre-feet capacity (or the Colorado River below Lake Mead) that are conducted pursuant to a federally-issued incidental take permit under the federal Endangered Species Act.
