Establishes the "I didn't pay enough fund" administered by the Department of Health Services, funded through voluntary taxpayer contributions from income tax refunds.

Allows taxpayers to designate a portion of their income tax refund as a voluntary contribution to the fund, with the designated amount subtracted from their refund and transferred to the fund.

Permits taxpayers to make additional donations to the fund in lieu of or in addition to refund designations by indicating this on their tax return.

Requires fund monies to be used equally: 50 percent for providing medicine to mentally ill persons in Arizona and 50 percent for providing medicine to persons under 18 years old suffering from chronic illness.

Repeals section 43-211 and amends section 43-621 of Arizona Revised Statutes, with retroactive effective date to taxable years beginning after December 31, 2011.
