Adds a new class 6 felony for knowingly altering a voter registration form without the registrant's consent in section 16-184.

Establishes a process for counties to remove voters from the permanent early voting list by December 1 of even-numbered years if they did not vote early ballots in both the primary and general elections for the two most recent general elections for federal office, with a 30-day confirmation notice required.

Requires permanent early voting list request forms distributed by candidates, political committees, or organizations to include a specific notice statement informing voters they are agreeing to receive early ballots for every election and will not vote at their assigned polling location.

Makes it a class 6 felony for any person who knowingly fails to submit completed permanent early voting list request forms by the submission deadline for the immediately following election.

Imposes civil penalties up to three times the production cost for forms that use an incorrect return address, and up to $25 per day for forms withheld from timely submittal.
