Parents may request written disclosure of identifying information (names, aliases, identification documents) regarding any de facto caregivers of their minor children.

Recipients of such requests must provide the requested information or written explanation within 30 days; if parents are not residing together, one parent must submit the de facto caregiver's name and contact information to the other parent within 30 days of request.

School and child care facility employees, and employees of properly licensed businesses providing brief child care services (such as retail stores and health clubs) are exempt from disclosure requirements.

A "de facto caregiver" is defined as a person age 18+ who supervises the child over 4 hours weekly, resides with the child part-time or full-time, or is in proximity to the child creating a tangent relationship for over 8 hours weekly.

Violation of disclosure requirements subjects a person to a civil penalty of $500.
