Mandates reports exclude patient identifying information and names; department must prepare annual statistical report available to the public while protecting individual identities.

Establishes penalties for facilities that willfully fail to file required reports or keep records, including license suspension or revocation for unprofessional conduct.

Classifies willful submission of false reports to the department as a class 1 misdemeanor and subjects violating organizations to civil penalties.
