Applicants must not have had a license revoked, voluntarily surrendered a license under investigation, or had disciplinary action imposed by another regulating entity.

Applicants must not have pending complaints, allegations, or investigations in another state relating to unprofessional conduct.

Reciprocity agreements with other states are permitted but may not allow out-of-state licensees to obtain licenses if they have not met standards substantially equivalent to or greater than Arizona's standards.
