Amends Arizona Revised Statutes Section 28-669 regarding accident report forms prepared and supplied by the department.

Requires written reports submitted by investigating officers to call for sufficiently detailed information about traffic accidents, including cause, existing conditions, and persons and vehicles involved.

Mandates that accident reports be made on forms approved by the department and contain all required information unless unavailable.

Directs the department to modify written accident report forms in coordination with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
