Authorizes Arizona cities, towns, or counties to establish voluntary programs providing emergency responders with critical health information about program participants involved in motor vehicle accidents or emergencies.

Requires program materials to include a yellow decal for the vehicle's rear driver's side window, a health information card with participant details (medical conditions, medications, allergies, emergency contacts), and a yellow envelope for the glove compartment.

Yellow decal on a vehicle alerts emergency responders to search the glove compartment for health information and authorizes responders to use the information to identify participants, assess medical conditions, contact emergency contacts, and consider preexisting conditions when providing treatment.

Allows cities, towns, or counties to charge program participants a nominal fee for program costs and requires distribution of materials to the public and law enforcement agencies.

Provides immunity from civil liability for emergency responders who act in good faith based on incomplete, incorrect, or outdated information provided on health information cards.
