Adds a new exception to Arizona's parental consent requirement for abortions on unemancipated minors: parental consent is waived if a physician or counselor provides specific pregnancy information and counseling to the minor.

Requires the counseling to cover pregnancy alternatives (carrying to term and keeping the child, adoption/foster care, prenatal/postnatal care), abortion procedures, birth control information, and the possibility of involving parents or guardians in the decision.

Mandates that the minor receive written notification that she may withdraw an abortion decision at any time before the procedure and that she may reconsider a decision not to have an abortion within the legal timeframe.

Requires the physician or counselor to have the minor sign a form documenting that all required information and counseling was provided, with the form kept on file by the attending physician.

The counseling exception applies in addition to two existing parental consent exceptions: cases involving sexual abuse by a family member or household contact, and cases where the minor's health requires immediate abortion.
