Requires the Department of Health Services, Arizona Health Care Cost Containment System (AHCCCS) Administration, and Governor's Office to collaborate on identifying goals and benchmarks to reduce diabetes incidence and improve care in Arizona.

Mandates submission of a biennial report (by January 15 of each odd-numbered year) to the legislature detailing the financial impact of diabetes on state agencies and political subdivisions.

Report must assess benefits of existing diabetes prevention and control programs, document funding sources and amounts, and describe coordination efforts between agencies on diabetes management and prevention activities.

Requires development of detailed action plans with specific steps to reduce diabetes and pre-diabetes impact, establish benchmarks for the upcoming biennium, and identify expected outcomes of proposed interventions.

Requires creation of a detailed budget identifying costs, resources, and implementation needs for the diabetes control and prevention plan.
